• Insurance and/or policy number.
• The date of the treatment.
• A brief description of the treatment.
• Performance code.
• The cost of the consultation.

 In practice, I collaborate with the following organizations for data processing, and have entered into processing agreements with them:

• Google.
• STRATO

Data Retention

In line with the Dutch Medical Treatment Agreement Act (Wet op de geneeskundige behandelingsovereenkomst  (WGBO) , I will retain your personal data for 20 years post the conclusion of your treatment.

Sharing of Data

With your consent, I may share your data with other healthcare providers for treatment purposes. This could include referrals to specialists or reporting to your primary care physician. I have established agreements with other healthcare providers to ensure the secure transfer of data.

Access, Amendment, and Deletion of Data
